The probe bodies formed over death of child Rafida Khan Raifa, who died at Max Hospital in Chattogram city for alleged wrong treatment and negligence of doctors, found doctors guilty and irregularities at the private hospital.
Raifa, the two years and four months child daughter of journalists Rubel Khan died in the hospital a week ago.
Two probe committees were formed following her death. The probe bodies submitted the reports.
One of the committees which was formed by the health ministry found a total of 11 irregularities in the hospital.
The committee was headed by Prof Dr Kazi Jahangir Hossain, Director Hospitals & Clinics and Line Director of Hospital services Management of Health Ministry.
In the report, the committee mentioned the irregularities, including the hospital license renewal without following due diligence, no appointment letter of physicians and nurses, no information about hospital staff, operating an unapproved pathological lab, absence of any blood bank and others.
The other three-member investigation committee headed by Civil Surgeon of Chittagong Dr Azizur Rahman also found negligence of physicians during the treatment of Raifa.
The committee found the doctors including Bidhan Roy, Debashish Sen and Shubra Dev guilty and recommended for taking action against the said doctors.
The committee also found irregularities of Max Hospital including lack of specialist doctors, trained nurses and dispute in treatment and admission of patients.
On June 29, Raifa was admitted to Max Hospital with a complaint of sore throat. She died from wrong treatment of doctors of the hospital, her parents alleged.

Chittagong Union of Journalists along with the social and political organizations has created a movement for the justice for Raifa to make the medical service pro-people in the port city.
